Put the image on the report. Right-click on it and select "Format picture...". Click on the formula button to the right of "Suppress" (DO NOT check the suppress check box!) Enter the conditional formula that should be true when you want the image to appear.

NOTE: You can't use a .ico file for this - you have to use an actual image file such as .jpg, .png, etc.
