	How do i get a form to request data from MSSQL server...

	What do you mean by a 'form'? Crystal Reports doesn't have the ability to display forms.

	if you are looking for a way to send SQL to a MS SQL DB there are a few tools OSQL SQLCMD WINSQL http://www.microsoft.com/downloads/details.aspx?FamilyID=c243a5ae-4bd1-4e3d-94b8-5a0f62bf7796&DisplayLang=en Access via ODBC Excel via MSquery / ODBC You should check with your local IT department.
